Encompass Health Home



What is a Children's Health Home?

A Health Home isn't a home at all. It consists of a network of providers, health plans and community-based organizations that work with you and your child to meet your child's needs. Whether it's physical/dental needs, mental health needs or support making your lives better, we can help.

Who can join?

  • Is your child ages birth to 21 and enrolled in Medicaid?
  • Does your child have chronic health issues such as asthma, diabetes or cardiac disorders?
  • Does your child have one of these diagnoses: HIV/AIDS, Serious Emotional Disturbance (SED) or Complex Trauma?
  • Are you having trouble meeting your child's health needs? Do you feel alone or in need of help? 

Why Encompass?

Encompass Health Home is a collaboration of Catholic Charities and their partners across Upstate New York managed by Catholic Charities of Broome County and supported by the Dioceses of Buffalo, Rochester, Syracuse and Albany. Encompass has a 100-year legacy of service in Upstate New York and a team of skilled and dedicated staff working with children and families. We respect and value all families. Every family is unique and has strengths; together we work to help you achieve your goals.

Encompass is an evolving and growing network of over 100 care management agencies throughout 48 counties experienced in serving individuals through a diversity of services across all ages. It is led by Catholic Charities in each of five regions: Buffalo, Rochester, Syracuse, Albany and the Norther Region.
