Schedule client appointments; conduct prescreening, as appropriate, in person or by phone; checks in clients for appointments.
Provide orientation information to new applicants.
Completes household information and determines income eligibility for WIC participants.
Conducts nutrition risk assessment to determine all nutrition risk criteria and program eligibility.
Prescribes,
tailors, authorizes food packages with the exception of contract infant
formulas that require medical documentation, exempt formulas and
special needs - Food Package III.
Provides nutrition education responsive to the identified needs/interests of participants.
Participates in the WICSIS trainings and updates. Becomes proficient at WICSIS computer tasks.
Obtains any medical data needed. Contacts health care providers.
Reviews WIC Medical Referral Form and any other related paperwork to ensure adequate documentation of health status.
Performs height and weight measurements, and hemoglobin determinations.
Encourages breastfeeding and makes referrals, participates in outreach activities including phone contact, meeting attendance and other activities.
Provides and documents appropriate follow-up referrals to other health and social services.
Completes documentation of Individual Contact History.
Participates in the WICSIS trainings and updates. Becomes proficient at WICSIS computer tasks.
Participates in all staff meetings and trainings.
Assists in preparation of reports, such as data collection, client surveys.
Meets quantitative expectations established by the agency.
Transports WICSIS laptop computers and other materials/equipment to temporary sites.
