Western New York Alzheimer's Caregiver Partnership

Comprised of area care and services professionals, this Partnership is dedicated to connecting caregivers of persons with dementia to the support and services they need to prevent caregiver burden and burnout - increasing the quality of life for the individuals we serve.

If you are a Caregiver, we can help!
We assist caregivers of persons with Alzheimer's or other dementia.There are no age requirements or age limits.

Grant monies provided by the NYS Department of Health, and administered through the WNY Alzheimer's Caregiver Partnership, fund and/or supplement the following:

  • Person Centered Care Consultation and Needs Assessment

  • Caregiver Support Groups

  • Powerful Tools for Caregiver Education Courses

  • Dementia Specific Caregiver Education Classes

  • Episodic Respite in a Social or Medical Day Program

  • Planned Overnight Respite Assistance

  • In-home Companion Services

  • Joint Enrichment Events and Programs

  • Transportation to/from programs, doctor's appointments, events

Members of the WNY Alzheimer's Caregiver Partnership, in addition to Catholic Charities of Buffalo, are the Alzheimer's Association, Western New York Chapter; and the Offices for the Aging of Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara and Wyoming counties.
